1 Year of Heavy Resistance Training Preserves Leg Strength 4 Years

Most adults lose strength fast after 60. The drop is not a mystery. Skeletal muscle mass falls 3 to 8 percent per decade after age 30. After 50, leg strength declines 12 to 15 percent per decade. By the eighth decade, total muscle mass is down by roughly half from young-adult levels.
For older adults in London, the cost shows up as slower walking, fewer stairs without a rail and rising fall risk. Sarcopenia, the clinical loss of muscle and strength, affects 4 to 25 percent of older free-living adults in the UK. The fix is well known. Strength training preserves muscle. The open question has been simple. How long does the benefit last after a structured programme ends? A Danish team gave a clear answer in 2024.

The LISA trial design
The LIve active Successful Ageing study (LISA) randomised 451 adults at retirement age into three arms for one year:
- Heavy resistance training (HRT): 3 supervised sessions per week at 70 to 85 percent of one-rep max
- Moderate-intensity training (MIT): home and circuit-style work using bands and bodyweight
- Control (CON): no exercise intervention
After the supervised year ended, participants returned for testing at 2 and 4 years post baseline. By the 4-year mark, 369 people remained in follow-up. Mean age was 71. Sixty-one percent were women.
The result
The HRT group held baseline isometric leg strength at four years (149.7 Nm at start to 151.5 Nm at four-year follow-up). The control group declined significantly. The moderate-intensity group sat between the two with no significant gain or strong protection.
One year of heavy work. Four years of preserved leg strength. Without continued supervision.

Why heavy beat moderate
Heavy loads recruit high-threshold motor units. These motor units control the largest, fastest muscle fibres. Older adults lose these fibres fastest with age. Light loads do not reach the threshold needed to train them. Moderate circuits preserve general fitness but leave the strength reserve untouched. The four-year gap between groups is the practical proof. The HRT group built a higher strength ceiling in year one. They then declined from a higher start. The CON group had no buffer. The MIT group built a small buffer which faded over time.
What heavy means in practice
Heavy in the LISA trial meant working at 70 to 85 percent of one-rep max. In plain terms, the last 2 to 3 reps of a set should feel hard but controlled. Form holds. The bar slows but does not stall. The NHS recommends two strength sessions per week for adults over 65. The Age and Ageing 2022 prescription for sarcopenia is two sessions per week, 1 to 3 sets per exercise, 6 to 12 reps at high effort, covering both upper and lower body.
A 2-session weekly template
Day A: Leg press 3 x 8 at heavy load, chest press 3 x 8, seated row 3 x 8. Rest 2 minutes between sets.
Day B: Goblet squat or leg extension 3 x 10, lat pulldown 3 x 8, shoulder press 3 x 8, hip hinge or Romanian deadlift 3 x 8.
Add weight every 2 to 3 weeks. Stop a set with 2 reps in reserve. Train each muscle hard enough to feel it the next day, not the next week.

Light circuits hold the line on general fitness. Heavy training builds a strength reserve you keep for years. If you are between 55 and 70, the LISA data suggests one structured year now will pay back well into your 70s. The earlier you start the heavy work, the longer the buffer lasts.
