Muscle Strength Predicts Lifespan: 2026 JAMA Evidence

Most people track health through weight, blood pressure or cholesterol. A February 2026 study in JAMA Network Open adds a stronger predictor of how long you live. Muscle strength.
Researchers at the University at Buffalo followed 5,472 older women for around 8 years. Women with the highest grip strength had a 33 percent lower risk of death than those with the lowest. Women who completed a chair stand fastest had a 34 percent lower risk than the slowest. The differences held after adjusting for total physical activity and sedentary time.

What the 2026 JAMA study found
The Buffalo team measured grip strength using a hand dynamometer and chair stand time using a simple sit-to-stand test. Women in the highest grip strength group held more than 24 kg of force. Women in the lowest held less than 14 kg. The mortality gap held after the team adjusted for accelerometer-measured physical activity, sedentary time and other risk factors. Strength carried information about survival beyond what total exercise volume captured.
Chair stand speed showed the same pattern. The fastest group completed a sit-to-stand in 11 seconds or less. The slowest took 17 seconds or more.
Why muscle strength predicts how long you live
Skeletal muscle is a metabolic organ. It pulls glucose from the blood after meals, releases anti-inflammatory signals during contraction, holds bones in alignment, and absorbs the impact of falls. As muscle thins, every one of those protective effects weakens. Sarcopenia, the clinical loss of muscle and strength with age, affects 5 to 13 percent of people aged 60 to 70 and 11 to 50 percent of people over 80 (Cruz-Jentoft 2019).
Stronger muscle also predicts better recovery from illness, surgery and falls. Hip fracture survival is far higher in adults with preserved lower-body strength. Sarcopenic adults spend more days in hospital and more weeks in rehab.

What actually builds the kind of strength linked to survival
Resistance training is the only intervention with consistent, repeated evidence for preserving and rebuilding strength across the lifespan (Fragala 2019 NSCA position statement, Cruz-Jentoft 2019 EWGSOP2 recommendations). Resistance training means any exercise that works the muscles against meaningful load: barbells, dumbbells, resistance machines, bands, or bodyweight with progressive difficulty.
Two to three sessions a week, covering the major muscle groups, is the general dose. Work at an effort where the last two reps feel hard but controlled. Progress load every 2 to 3 weeks. Sleep and protein matter alongside the training.
Two strength tests you can do today
Grip strength: a hand dynamometer (£20 to £40 online) gives a quick reading. Women in the LaMonte cohort with grip force above 24 kg had the lowest mortality. Men should aim above the European Working Group on Sarcopenia threshold of 27 kg.
Chair stand: sit in a straight-backed chair with arms crossed over the chest. Stand fully, then sit, without using arms. Time five repetitions. Less than 12 seconds is healthy. Over 15 seconds indicates declining lower-body strength.
Practical takeaways
Add two resistance training sessions a week starting this week. Pick five exercises covering squat, hinge, push, pull and carry. Add a small amount of weight or one extra rep each week. Test grip and chair stand every three months. The 2026 JAMA evidence does not mean you need to lift heavy in your 70s. It means you should build the strength reserve now while you are younger.
