Around 45 out of every 100 dementia cases might be prevented or delayed by changing everyday habits. The 2024 Lancet Commission on dementia reached that figure after reviewing 14 modifiable risk factors across the life course. One of those factors is physical inactivity. That places movement among the levers you control for your own future brain health.
This matters for London adults who sit through long desk days and busy commutes. You do not need a special programme or a lab test. The research points to walking, and to a mix of aerobic and strength work, as practical ways to lower risk. Below is what the strongest studies show, and how to turn the findings into a simple weekly routine.
Why movement protects the ageing brain
The brain relies on a healthy blood supply. Regular aerobic activity supports the small vessels that feed brain tissue and keeps blood pressure in a lower range. This links back to dementia risk, because damaged vessels and raised pressure in midlife increase the odds of later cognitive decline. The Lancet Commission lists hypertension, diabetes and physical inactivity as separate but connected risk factors, and exercise acts on all three.
Movement also appears to support the hippocampus, the brain region tied to memory. Trials in older adults have measured better performance on memory and attention tasks after several weeks of structured exercise. The exact biology is still being mapped, and researchers are careful to say the effect sizes are modest rather than dramatic. The consistent signal across many studies is what makes the case strong, not any single result.
Two more routes are worth naming. Regular activity improves how the body uses insulin, which lowers the chance of type 2 diabetes, itself a named dementia risk factor. Activity also lowers chronic inflammation, a process tied to brain ageing in long-term studies. None of these routes works in isolation. They stack, which helps explain why active people show lower risk across many different populations and study designs.
What the step-count research shows
A 2022 study in JAMA Neurology followed 78,430 UK adults aged 40 to 79 who wore a wrist tracker. Over almost 7 years, higher daily step counts were linked to lower dementia risk in a clear dose-response pattern. Around 3,800 steps a day was linked to a 25 percent lower risk, and near 9,800 steps a day to roughly half that risk compared with very low activity. Pace added something on top of volume. Adults whose fastest 30 minutes reached about 112 steps a minute, a brisk walk, showed an even stronger link, near 62 percent lower risk in that part of the analysis.
Two points keep this fair. First, these are observational associations, so they show a strong link rather than final proof of cause. Second, the pattern held after the researchers accounted for age, sleep, diet, smoking and other habits, which strengthens the case. For most people the practical read is simple. More daily steps, and some of them brisk, track with a lower chance of dementia.
Walking is not the only route
Steps are an easy place to start, and they are not the whole picture. A 2018 meta-analysis in the British Journal of Sports Medicine pooled 39 controlled trials in adults over 50. Structured exercise improved scores on tests of memory, attention and processing speed. Both aerobic training and resistance training helped, and the authors recommended sessions of at least moderate intensity on as many days as feasible. Strength work carries a second benefit for older adults, because it helps preserve muscle and balance, which keeps people active and independent for longer.
This fits official guidance. The World Health Organization recommends at least 150 minutes of moderate aerobic activity a week, or 75 minutes of vigorous activity, for adults including those over 65. Its 2019 guideline on cognitive decline names physical activity as a recommended step to lower dementia risk. The message across the research and the guidance lines up. A weekly mix of walking, some brisk effort and two strength sessions covers the ground.
Consistency beats intensity here. The people in these studies were not endurance athletes. They were ordinary adults who moved most days across years, not weeks. A pattern you keep for a decade shapes brain ageing far more than a short burst you abandon by spring. Pick activities you tolerate and enjoy, then hold them steady. The broader review of physical activity and dementia pooled 58 cohort studies and still found a 20 percent lower risk in more active adults, which points to a real and repeatable effect rather than a one-off finding.

The most common error is treating this as all or nothing. The step data shows benefit well before 10,000 steps, so a rise from 3,000 to 5,000 is worthwhile on its own. A second error is skipping strength work because walking feels enough. The cognition trials included resistance training for a reason. A third is starting too fast after years of low activity, which raises the chance of a strain or a sore joint. Build the load over weeks.
