Ultra-Processed Foods: What 10 Million People Reveal

508 extra calories a day. Same person, same appetite, different food. In a controlled trial, adults lived on a research ward for a month. Their meals matched for calories, sugar, fat, salt and fibre. The only difference was processing. On the ultra-processed menu, people ate 508 more calories a day and gained weight. On the unprocessed menu, the same people stopped overeating.

Ultra-processed foods now supply more than half of the calories many adults in the UK eat. A review of nearly 10 million people links higher intake to earlier death, heart disease, type 2 diabetes and poorer mental health. The evidence is strong enough to change how you shop, and the fixes are practical.

What counts as ultra-processed food

Researchers sort food by how much it is processed, using a system called NOVA. Group one is whole or minimally processed food: vegetables, fruit, beans, eggs, fish, meat, plain milk, oats. Group two is kitchen ingredients like oil, butter and salt. Group three is processed food: tinned fish, cheese, fresh bread. Group four is ultra-processed food.

Ultra-processed foods are industrial formulations built from refined ingredients and additives you would rarely keep in a home kitchen: emulsifiers, sweeteners, colours, flavour compounds and protein isolates. Common examples include fizzy drinks, packaged snacks, mass-produced sliced bread, most breakfast cereals, reconstituted meat products, instant soups and many ready meals. Not every ultra-processed food carries the same risk, and a wholegrain supermarket loaf is not the same as a sugary energy drink. The pattern across the diet is what matters most.

What the evidence shows

The largest summary so far is a 2024 BMJ umbrella review by Lane and colleagues. It pooled 14 meta-analyses covering nearly 10 million people. Higher ultra-processed intake showed a direct link to 32 of 45 health outcomes, spanning death, heart disease, metabolic health, the gut and mental health.

The strongest grade of evidence, which the authors call convincing, pointed to about a 50% higher risk of heart-disease death, a 48 to 53% higher risk of anxiety and common mental disorders, and a 12% higher risk of type 2 diabetes. A second tier, graded highly suggestive, linked higher intake to a 21% higher risk of death from any cause and a 22% higher risk of depression.

A 2025 dose-response meta-analysis put the same comparison a little lower, at roughly 15% higher risk of death from any cause for the highest intake, and mapped how risk climbs step by step as intake rises. The two reviews differ in size but point the same way. The pattern held across countries, age groups and both sexes, which is a sign the link is consistent rather than a quirk of one population.

These foods also crowd out better options. A plate heavy in packaged snacks and ready meals leaves less room for the fibre, protein and micronutrients that whole foods bring. So the harm works in two directions at once: more of the foods tied to risk, and less of the foods tied to protection.

Why these foods are easy to overeat

The NIH ward trial gives the clearest answer. Hall and colleagues fed 20 adults two diets in turn, each for two weeks, matched for calories on offer, sugar, fat, salt, fibre and macronutrients. People still ate 508 calories more per day on the ultra-processed diet and gained weight, then lost the weight on the unprocessed diet. Appetite tracked the food, not the person.

Several features drive this. Ultra-processed foods pack more calories into each bite, so the same volume delivers more energy. Soft textures speed up eating, and faster eating means more calories before the body signals fullness. Engineered combinations of fat, sugar, salt and flavour push you to keep going past the point you would stop with plain food. The result is a quiet daily surplus you do not notice meal to meal.

Association, cause, and what stays uncertain

Most of the large numbers come from observational studies. They track what people report eating and follow their health over years. People who eat the most ultra-processed food often differ in income, smoking, sleep and total diet quality, and researchers adjust for these factors without removing them fully. So the review authors describe associations, not proof of cause for every outcome.

The ward trial is the bridge. It is small, with 20 people over four weeks, but it is a randomised test, and it shows a direct cause for the overeating behind weight gain and metabolic risk. Larger feeding trials are running now. Read together, the evidence supports a clear direction: lower the share of ultra-processed food on your plate.

Five ways to cut back

1. Build the plate on whole foods
Start each meal from vegetables, fruit, beans, eggs, fish, meat, plain dairy and wholegrains. When most of the plate is group-one food, ultra-processed extras take up less room by default.
2. Read the ingredient list, not the front
Front-of-pack claims like high protein or low fat tell you little about processing. Turn the pack over. A long list with colours, emulsifiers, sweeteners and names you would not cook with flags an ultra-processed product.
3. Fix breakfast first
Breakfast is often the most processed meal. Swap sugary cereal and pastries for oats, eggs, or plain yogurt with fruit. One swap, repeated daily, shifts a large share of weekly intake.
4. Cook once, eat twice
Batch-cook and freeze portions. A home-cooked meal from the freezer beats a ready meal on the night you have no time, which is when ultra-processed food usually wins.
5. Cut the biggest sources first
You do not need zero. Target the largest contributors in your week, usually sugary drinks and packaged snacks, before worrying about a slice of wholegrain bread.

Common mistakes

The first is chasing a zero ultra-processed diet and giving up after a hard week. The evidence is about lowering intake, not reaching perfection. The second is trusting health-marketed products. Many protein bars, flavoured yogurts and plant-based meats sit firmly in group four. The third is fixing solid food and ignoring drinks. Sugary soft drinks are among the most studied sources, and they add up fast.

Frequently asked questions

Are all processed foods bad for me?
No. Processing runs on a spectrum. Tinned beans, frozen vegetables, plain yogurt and fresh bread sit at the minimally or moderately processed end and fit a healthy diet. The concern is the ultra-processed end, where additives and refining replace whole ingredients.
Is ultra-processed food the same as junk food?
They overlap but are not identical. The category is broader and includes products marketed as healthy, such as some breakfast cereals, cereal bars and meat alternatives.
Do I have to cut it out completely?
No. The research links higher intake to higher risk, so the practical goal is to lower your share. Cutting the largest sources delivers most of the benefit.
Does the evidence prove these foods cause disease?
The bulk of the evidence is observational and shows strong associations rather than proof for every outcome. One inpatient randomised trial shows a direct cause for overeating and weight gain. More feeding trials are under way.
How do I spot ultra-processed food quickly?
Read the ingredient list. A short list of foods you recognise is a good sign. A long list with emulsifiers, sweeteners, colours and isolates points to an ultra-processed product.

The bottom line

Ultra-processed foods are built to be easy to eat and easy to overeat. The biggest review to date links a higher share of them to earlier death, heart disease, type 2 diabetes and poorer mental health, and a controlled trial shows how the overeating starts. You do not need a perfect diet. Anchor your meals on whole foods, read the label, fix breakfast, and cut your biggest sources first. Small, repeated swaps add up.
