Workout Variety Lowers Mortality Risk: 34-Year BMJ Study

You walk into the gym. You do the same routine you did last week. Same exercises, same order, same weights. Showing up matters. A 2026 BMJ Medicine analysis of more than 111,000 adults tracked for 34 years suggests your body wants something more. Variety.
Adults with the widest mix of exercise types had a 19 percent lower risk of dying from any cause than those with the narrowest. The effect held after researchers adjusted for total exercise time. Two people doing the same amount of weekly exercise saw different mortality outcomes when one chose variety and the other chose repetition.

The problem with doing the same thing
Most adults find one activity they like and stick with it. Runners run. Lifters lift. Walkers walk. Any of these reduces your risk of early death versus doing nothing. The trade-off is everything else goes untrained. Heart and lungs get strong but muscles weaken. Or legs build strength while balance falls behind. Over decades, the imbalance compounds.
The Han 2026 paper drew on two large prospective cohorts, the Nurses' Health Study II and the Health Professionals Follow-up Study. Combined, they tracked 111,467 US adults for up to 34 years of follow-up. Researchers measured the number of distinct activity types each person reported, then linked the variety score to mortality after adjusting for total exercise minutes, age, smoking, diet quality and other lifestyle factors.

Why variety matters on its own
Different activities load different systems. Aerobic work (walking, cycling, swimming) trains the heart and lungs. Resistance work (weights, bands, bodyweight) builds muscle and bone density. Balance and mobility work (yoga, tai chi) protects joints and lowers fall risk. A 2019 meta-analysis of 370,256 people found combining resistance with aerobic exercise lowered all-cause mortality by 40 percent, compared with 21 percent for resistance alone (Saeidifard 2019).
The WHO recommends 150 to 300 minutes of moderate aerobic activity a week plus muscle-strengthening on two or more days. The Han 2026 finding adds a third layer: spread your effort across activity types.
How to add variety this week
Keep your favourite activity. Add one or two different ones. A practical pattern for a London adult:
- Aerobic anchor: two brisk walks, runs or cycle rides of 30 to 45 minutes.
- Strength anchor: two resistance sessions covering squat, hinge, push, pull and carry.
- Mobility anchor: one yoga, Pilates, swim or mobility session of 30 to 45 minutes.
This spread covers all three load types in around 4 to 5 hours a week. The Han 2026 data suggests this beats five hours of the same activity for long-term survival.
Honest limits of the evidence
The Han 2026 study is observational. People who pick varied exercise also tend to be more health-conscious in other ways. Adjustments helped, but residual confounding stays. The biological story (different systems loaded by different activities) is well established, so the direction of effect is plausible. The exact size of the variety bonus is the part to hold loosely.
Practical takeaways
Hold your favourite activity in place. Add one new movement pattern this week. Aim for at least three distinct activity types across the week: aerobic, strength, and mobility or balance. Walking counts. Gardening counts. Bodyweight circuits at home count. The goal is range, not novelty for its own sake.
