Strength Training Benefits: What the Research Shows

Strength training is the most underrated exercise modality in modern health. Adults lose 3 to 8 percent of muscle mass per decade after age 30 if they do not train it. The losses accelerate after 60. The NHS guidance for adults aged 19 to 64 calls for muscle-strengthening activity on at least 2 days a week, working all major muscle groups. Most adults meet the walking target and miss the strength target. The cost shows up as falls in later life, reduced metabolism, declining insulin sensitivity, lower mood, and the slow physical narrowing that quietly limits independent living.
The 2019 European Journal of Preventive Cardiology meta-analysis (Saeidifard et al., PMID 31597479) pooled 11 cohort studies and found regular resistance training was associated with a 21 percent lower all-cause mortality risk, independent of aerobic activity. Two short sessions a week is enough to capture most of the effect.

What strength training actually does
Muscle and bone. Mechanical load triggers muscle protein synthesis and stimulates the bone-forming osteoblast cells. Two strength sessions a week for 12 weeks adds 1 to 2 kg of lean tissue in untrained adults and raises bone mineral density at the hip and spine.
Mortality. The Saeidifard 2019 meta-analysis is the strongest single piece of evidence: 21 percent lower all-cause mortality with regular resistance training. The 2022 BJSM meta-analysis (Momma et al.) added that combining muscle-strengthening activity with aerobic activity gave the largest reduction in all-cause and cardiovascular mortality.
Glucose control. Skeletal muscle is the largest site of post-meal glucose disposal. More muscle and more frequent contraction equals better insulin sensitivity. A 2017 BMC Public Health study found resistance training reduces type 2 diabetes risk by roughly 30 percent.
Falls prevention. Lower body strength is one of the strongest predictors of falls risk in older adults. Sherrington and colleagues (Cochrane 2019) found exercise programmes incorporating progressive resistance training reduced falls by 23 percent in community-dwelling older adults.
Mood and mental health. The 2018 JAMA Psychiatry meta-analysis by Gordon et al. (PMID 29800984) found resistance training produced moderate reductions in depressive symptoms across 33 randomised trials, an effect size comparable with low-dose antidepressants.
A simple weekly plan
Total weekly commitment: 90 to 120 minutes in two sessions. Add weight or reps each week. When you stall, deload to 80 percent of the previous best for one week then rebuild. Track the main lift weight and rep count in a simple notebook or app. Across a year, most adults add 30 to 60 kg to their compound lifts and reshape their daily function.
Common myths about strength training
- "Cardio first, weights second." Aerobic exercise and strength work cover different pathways. Both matter, neither replaces the other. NHS guidance specifies both for a reason.
- "Women shouldn't lift heavy." Women produce far less testosterone than men and do not develop bulk easily. Two strength sessions a week produces lean athletic muscle, not bulk. The bone-density argument is even stronger for women given osteoporosis risk after menopause.
- "It's bad for your joints." Progressive resistance training strengthens joint-supporting structures (ligaments, tendons, cartilage). Knee osteoarthritis trials specifically use it because it reduces pain and improves function.
- "I'm too old to start." Adults in their 80s have produced measurable strength and lean mass gains in trial settings. The earlier you start the easier it is, but no age makes the work pointless.
- "You need a gym." A few resistance bands, a kettlebell or pair of dumbbells, and bodyweight movements cover the entire NHS strength target at home. Gyms make heavy progression easier; they are not required for the health gains.
The protein partner
Training without protein leaves results on the table. Morton et al. (Br J Sports Med 2018, PMID 28698222) pooled 49 studies and 1,863 adults and found protein supplementation added an extra 2.49 kg of 1RM strength and 0.30 kg of fat-free mass during prolonged resistance training. Returns plateaued above 1.62 g per kilogram of bodyweight per day. For a 75 kg adult that is around 120 g of protein daily. Spread across three or four meals. Eggs, fish, lean meat, dairy, beans, and plant-based protein blends all count. The amount matters more than the source.
