Japanese Walking: The 30-Minute Interval Method That Lowers Blood Pressure

Most general walking advice asks for 30 to 60 minutes a day at a steady pace. For sedentary adults, that ask is wide enough to fail. Japanese Interval Walking Training (IWT) developed at Shinshu University offers a structured alternative. Three minutes fast, three minutes slow, repeat five times. Total 30 minutes. Four sessions a week. The trials show meaningful drops in blood pressure, real gains in leg strength and around 14 percent rises in peak aerobic capacity.

The method comes from the work of Professor Hiroshi Nose and Associate Professor Shizue Masuki, building on the original randomised trial by Nemoto and colleagues published in Mayo Clinic Proceedings in 2007. The protocol has been refined and replicated across more than a decade of follow-up research.

What the original trial found

The 2007 Nemoto trial randomised 246 middle-aged and older adults (mean age 63) to one of three groups for 5 months: no walking, moderate-intensity continuous walking, or interval walking training. The IWT group walked 5 sets of 3 minutes at around 70 percent of peak aerobic capacity, alternating with 3 minutes at 40 percent. Sessions were 30 minutes, target 4 days per week.

Compared with the no-walk group, the IWT arm dropped systolic blood pressure by around 9 mmHg, improved knee extension and flexion strength by 13 to 17 percent, and lifted peak aerobic capacity by 14 percent. The moderate steady-pace walkers showed smaller, non-significant changes on the same outcomes.

Why intervals beat steady pace

Steady moderate walking lifts heart rate into a comfortable zone, then stays there. Interval walking pushes heart rate higher for short, repeated bouts. Those harder bouts recruit fast-twitch muscle fibres, which sedentary adults lose first with age. The slower recovery phases let breathing and heart rate fall before the next push.

The result is a stronger stimulus for the cardiovascular system, the leg muscles and the mitochondria, without the joint impact of running. A 2024 review by the Shinshu team confirms the protocol also improves glycaemic control in adults with type 2 diabetes by enhancing glucose uptake during the hard bouts.

The exact protocol

No equipment. No gym. Any path or pavement works. A free interval timer app on your phone will run the alerts.

  • Warm up at your normal pace for 5 minutes.
  • Walk fast for 3 minutes. "Fast" means short, clipped sentences only (around 70 percent max heart rate).
  • Walk slow for 3 minutes at a comfortable, easy pace (around 40 percent max heart rate).
  • Repeat the fast / slow block five times (30 minutes total).
  • Cool down at an easy pace for 3 to 5 minutes.

Frequency: four sessions per week. Three sessions delivers most of the cardiovascular and blood pressure benefit but the leg-strength gains scale with frequency, with five days a week producing roughly 15 percent more improvement.

Starting if you are very sedentary

Begin with two rounds of intervals instead of five. Build to four rounds over two weeks. Walk on flat ground for the first fortnight before adding hills. If you have heart disease or uncontrolled high blood pressure, consult your GP before starting. The harder bouts are safe for most adults but medical history matters.

When to expect results

Measure resting blood pressure before you start and again at 4 to 6 weeks. Most adults see a 4 to 8 mmHg drop in systolic BP by the 6-week mark. Leg strength gains follow at 8 to 12 weeks. Peak aerobic capacity rises gradually over 3 to 5 months.

Practical takeaways

Pick four days a week. Set a phone timer for 3-minute alerts. Walk for 30 minutes total. Track resting blood pressure once a week. Add a hilly route once you can hold the pace cleanly. Pair the IWT with two short resistance sessions a week to protect muscle mass.
